Endometriosis, adenomyosis, or fibroids? 🩺🩸🧩

You may have been told you have endometriosis, adenomyosis, fibroids, or “possibly all three,” and instead of feeling relieved, you may feel even more confused.

Because when you are already living with heavy bleeding, deep pelvic pain, pressure, bloating, fatigue, bowel symptoms, bladder symptoms, pain during intimacy, or periods that take over your whole life, another medical word can feel like another weight on your chest.

I want to say this gently. You are not silly for being confused. These conditions can overlap, the symptoms can sound similar, and sometimes your body does not fit neatly into one simple box.

Endometriosis means tissue similar to the lining of the uterus is growing outside the uterus. It may be found on the ovaries, pelvic lining, bowel, bladder, ligaments, or other areas. This is why your pain may not feel like “period pain” only. You may notice pain with bowel movements, bladder pressure, back pain, hip pain, pain with s*x, fatigue, nausea, or flares that seem to affect your whole body.

Adenomyosis is different. With adenomyosis, tissue similar to the uterine lining grows into the muscle wall of the uterus itself. In simple words, the problem is inside the wall of the womb. This can make the uterus feel bulky, tender, swollen, or heavy. You may notice very heavy bleeding, large clots, intense cramping, pelvic heaviness, bloating, or that awful “dragging” feeling that makes standing, walking, or functioning harder.

Fibroids are different again. Fibroids are non-cancerous growths made from muscle and fibrous tissue that grow in or on the uterus. Depending on where they sit and how large they are, they may cause heavy bleeding, pressure, frequent urination, constipation, back pain, pelvic fullness, or fertility concerns.

A simple way to remember it is this:

• Endometriosis is usually outside the uterus
• Adenomyosis is within the muscle wall of the uterus
• Fibroids are growths in or on the uterus

But here is what many women are not told clearly enough: you can have more than one of these at the same time.

This matters because if one condition is found, it does not always explain everything. Your body might be trying to tell you there is more going on. If you still have severe pain after treatment, if your bleeding feels extreme, if your bloating feels constant, if you feel pelvic pressure that nobody has explained, or if your symptoms are affecting your work, relationship, sleep, intimacy, or mental health, you deserve deeper answers.

This does not mean you need to panic. It means your symptoms deserve proper attention.

Here is what can help when you speak to a doctor or specialist:

• Write down where the pain is, not just how strong it is
• Track bleeding, clots, bowel symptoms, bladder symptoms, and fatigue
• Mention pain during or after s*x if it happens
• Ask whether imaging has checked for adenomyosis or fibroids
• Ask whether deep endometriosis has been considered
• Bring photos or notes if your bleeding or bloating is hard to explain
• Say clearly how much your symptoms affect your daily life

One small thing you can try today is to write a “symptom pattern” note in your phone. Not a perfect diary. Just a simple list of what happens before, during, and after your period, and what symptoms appear even when you are not bleeding.

Because sometimes the most powerful thing you can bring into an appointment is not just pain, but a pattern.

You deserve to understand what is happening in your body. You deserve more than being told “it is normal” when your life is being interrupted. You deserve a doctor who listens, investigates, explains, and treats you like your quality of life matters.

And please remember this: needing answers does not make you difficult. Asking questions does not make you dramatic. Wanting your pain taken seriously does not make you negative. It means you are trying to survive inside a body that has been asking for help.

Save this for the next time you need a clear reminder of the difference between endometriosis, adenomyosis, and fibroids. Share this with a woman who keeps being told her symptoms are “just periods,” when deep down she knows they are more than that.
